- If you have a paper form, your 8-digit ID is printed on your survey.
- If you cant find your 8-digit ID, send us an e-mail with your name and date of birth. We will reply with your ID as soon as possible.
This questionnaire will take around 30 minutes to complete. We encourage you to complete the questionnaire in one sitting, but if you get disconnected or have to leave, your answers will be saved. To resume and complete the survey you can re-enter through the email letter or on our website (but you will need to login with your 8-digit ID and date of birth).
